Please provide a detailed hair color history for up to 10 years. I need specific details, such as what was used, when, and where. It's imperative to tell me everything. If no color has been used, Submit N/A

What are the concerns you are currently experiencing with your hair? Briefly explain the length and density.

What is your availability? (I am currently not accepting weekend or evening appointments).

I am no longer accepting black box color corrections.

Lastly, I will need your full name, phone number, and email. I do require a $20 nonrefundable fee at the time of booking for new clients.

I ask you to pay your deposit via square invoice, Venmo (@stephanieslayshair), or cash before your account is on the books.
